Meeting the Market’s Rising Demand for Value-Based Care

An increased focus on improving patient outcomes coupled with an aging population has created demand for value-based senior primary care solutions. With physician shortages sweeping the nation, we believe that we must rely on a highly qualified, untapped and overlooked workforce, Nurse Practitioners. NPs are well positioned for senior healthcare and can step into primary care provider roles, but oftentimes lack the platform, model, and infrastructure. We’re here to change that.

Our Clinical Programs

Enabling Value-Based Care with Nurse Practitioners

We have two unique business models that help our partners begin or accelerate their transformation to value-based care. Through both of these models, we aim to deliver exceptional patient experiences and outcomes.

Integrated Clinical Programs

We partner with risk-bearing organizations to support them in delivering value-based care. We design, deploy, and manage a range of services, including risk adjustment, care transitions support, complex care management, and quality improvement. Our specially trained care teams and experienced operators lead these clinical programs.

Primary Care

In partnership with health plans, we build thoughtfully designed, senior-centric primary care practices where patients are paneled directly to Nurse Practitioners within value-based arrangements, with special consideration for Full Practice Authority states challenged by physician shortages and limited access to care.
